Drop-In Day, Summer Camp, and School Year Extra Forms

If your child has allergies or will be receiving medication while at CHCNS, please fill out the related form and:

1) Submit one copy via the Jovial Family Portal:

Access your account in the Jovial Family Portal. You should have received an invitation to join via e-mail after your initial forms were processed. Or follow this link: https://www.jovial.org/chcns/family
On the left hand menu, click the “forms” or “requirements” tab.
Print, fill out, and upload an image of the required forms:

2) Print out one copy and place it in a ziplock bag with your child's medication if applicable.

Duty Parent/Caregiver Forms (SCHOOL YEAR ONLY)

For the Cooperative Model Program, families are required to serve Duty Days, assisting the Lead Teachers. While we love when parents can serve, families may send an approved caregiver to serve in their stead (nanny, grandparent, etc.). ALL Duty Day Volunteers must fulfill the requirements set forth by the DC Office of the State Superintendent of Education. These requirements are extensive and require time to complete before the start of the school year. This is for the safety and well-being of all CHCNS students as well as the volunteer.

While student forms should be uploaded to the family's Jovial account, volunteer forms are best forwarded as a file or JPG directly to the School Administrator. Forms can be filled out digitally or printed out, filled, and scanned. Once a family has enrolled in the school year program, they will be given access to the list of forms and requirements for the following school year. They should be fulfilled and forwarded to info@chcns.us.
